About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Home not Hospital. As a Support Worker, you will play a vital role in providing high-quality care and support to an individual with learning disability, autism, and mental health needs.

Key Responsibilities
  • Provide person-centered support to the individual, promoting their independence and well-being.
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores.
  • Support the individual in developing and maintaining daily routines and habits.
  • Encourage and facilitate social interactions and community engagement.
  • Monitor and report any changes in the individual's behavior or well-being to the management team.
Requirements
  • Experience working with individuals with learning disability, autism, and mental health needs.
  • Good understanding of Autism and its impact on daily living.
  • Ability to work as part of a team and provide support during challenging situations.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and adhere to data protection policies.
