About the Role
We are seeking a skilled Memory Assessment Nurse to join our team in Nottinghamshire Healthcare NHS Foundation Trust. This exciting opportunity is part of our Mental Health Services for Older People directorate, where you will work closely with a multi-disciplinary team to provide assessment, diagnosis, and support for individuals aged 65 and over with cognitive impairment.
As a key member of the team, you will be responsible for reviewing referrals, conducting initial assessments, creating treatment plans, and signposting patients to post-diagnostic support. Your expertise in assessing and prescribing for people with dementia will be invaluable in this role.
This position offers a unique chance to work in a dynamic environment, contributing to the development of our services and shaping the future of mental health care for older adults.
Key Responsibilities
- Conduct thorough assessments of clinical presentation and symptoms
- Develop and implement personalized treatment plans in collaboration with the MDT
- Liaise with healthcare professionals to explore symptoms and causes
- Signpost patients to relevant services and support networks
Requirements
- RNMH qualification and current registration with the NMC
- Masters degree or equivalent experience in a relevant field
- Substantial experience working in multidisciplinary teams
- Clinical supervision skills and ability to manage change
