Care Coordinator Specialist

Active Care Group is seeking a skilled Care Coordinator Specialist to join our team at Murdostoun Brain Injury Rehabilitation Centre.
We provide individualised assessment and intensive rehabilitation to help people recover from a brain injury. As a Care Coordinator Specialist, you will be responsible for planning, organising and implementing a social, recreational, and leisure activity programme in partnership with our existing Activities Co-ordinators.
You will work as part of a multi-disciplinary team and under your own initiative to create a stimulating environment that promotes independence and quality of life for our residents.
Key Responsibilities:
- Plan and deliver individual and group activities within the unit and the wider community.
- Collaborate with the multi-disciplinary team to ensure a holistic approach to care and rehabilitation.
- Develop and maintain a range of activities that cater to different interests and abilities.
- Ensure the safety and well-being of residents during activities.
