We are seeking a highly skilled and motivated Independent Living Coordinator to join our team at Social Care in Action. This is an exciting opportunity to make a real difference in the lives of individuals who require personalized support and assistance to maintain their independence.

In this role, you will be responsible for coordinating care and support services, ensuring that individuals receive tailored assistance to meet their unique needs and preferences. Your primary responsibilities will include:

  • Coordinating care and support plans with individuals, families, and healthcare professionals
  • Developing and implementing personalized support strategies to promote independence and participation
  • Maintaining accurate records and reporting progress to colleagues and management

We offer a competitive salary of £26,000 - £30,000 per annum, depending on experience, plus comprehensive training and ongoing support. Our benefits package includes paid holiday, refer-a-friend scheme, and access to a free Blue Light Card.
