The ideal candidate will be responsible for:
- Providing direct care and support to residents, including assisting with daily living tasks, administering medications, and promoting independence.
- Working collaboratively with other members of the care team to ensure seamless delivery of care and services.
- Engaging with residents, families, and healthcare professionals to promote effective communication and partnership working.
Key Responsibilities:
- Delivering person-centred care that respects the dignity and autonomy of each resident.
- Monitoring and reporting any changes in resident's condition or well-being.
- Participating in regular team meetings and contributing to quality improvement initiatives.
