About the Role

We are seeking a highly skilled and dedicated Senior Mental Health Practitioner to join our Home Treatment Team in Chester. As a key member of our team, you will be responsible for providing comprehensive assessments and interventions to individuals presenting in crisis.

The successful candidate will have a recognized qualification in mental health and a good amount of post-registration experience. You will be expected to work as part of a multi-disciplinary team, providing a comprehensive assessment of people presenting in crisis and using your clinical skills and knowledge to provide intervention and treatment.

The role involves working across Urgent Care Mental Health First Response Services, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services, and the Crisis Triage and Response Service.

Key Responsibilities
  • Conduct comprehensive assessments of individuals presenting in crisis
  • Develop and implement individualized treatment plans
  • Work collaboratively with other healthcare professionals to provide a comprehensive service
  • Provide education and training to other healthcare professionals

Person Specification

The successful candidate will have a recognized qualification in mental health, a good amount of post-registration experience, and a sound knowledge of current professional staff development and continuous service improvement issues. They will also have a willingness to undertake and complete nurse prescribing certificate or non-medical prescribing certificate.

The ideal candidate will have a good understanding of the Trust's Values and Behaviours, which are 'the 6 Cs' - Communication, Competence, Courage, Care, Compassion, and Commitment.
