About the Role

Your Responsibilities

  • Conduct comprehensive mental health assessments and develop individualized care plans for patients under your supervision.

  • Provide direct support and guidance to patients, acting as their primary point of contact in the absence of their designated care coordinator.
  • Facilitate key clinics within the Community Mental Health Team (CMHT), ensuring smooth operations and timely service delivery.
  • Contribute to a supportive and recovery-focused environment, promoting patient well-being and empowerment.

About You

You will be a Registered Mental Health Nurse with a proven track record of providing exceptional care in a clinical setting. Your strong clinical knowledge, understanding of mental health legislation, and experience in risk assessment and management are essential for success in this role.

Excellent communication and interpersonal skills are crucial to build rapport with patients and effectively collaborate within the team. You will be adaptable, self-motivated, and committed to delivering high-quality patient outcomes.
