Job Summary

We are looking for a Virtual Care Coordinator to join our team of healthcare professionals. The successful candidate will be responsible for coordinating virtual care services, ensuring seamless transitions between healthcare providers, and promoting effective communication among patients, families, and healthcare teams.

The ideal candidate will have excellent organizational and time management skills, strong attention to detail, and the ability to work effectively in a fast-paced environment. You will be responsible for managing electronic patient records, communicating with patients and healthcare teams, and identifying areas for process improvement.

This role requires a Registered Nurse with experience in a relevant specialty. You will be expected to stay up-to-date with the latest developments in virtual care and participate in quality improvement initiatives to enhance patient outcomes.
