Advanced Care Practitioner

The post holder will be an Advanced Practitioner, Prescribing Nurse Specialist, or Paramedic Clinician, working clinically to provide and develop the Home Visiting Service, while also providing clinical support to the PCN Frailty Team. The post holder will work closely with the PCN Leadership Team and wider Health and Care Partners.
The Home Visiting Service supports patients registered within Carlisle PCN, covering approximately 38,000 patients registered with three general practices: Eden Medical Group, Carlisle Central Practice, and Warwick Square Group Practice.
The Home Visiting Service provides a combination of acute on-the-day, urgent, and routine visits. Additionally, the post holder is expected to support the Frailty Team in providing a combination of visits covering acute, clinical review, chronic disease management, proactive caseload management, and care planning for the Frailty population. This includes weekly care home ward rounds to PCN-aligned care homes, nursing, and residential facilities.
The PCN Team works with health and care partners, including the NCIC trust, Adult Social Care, and voluntary sector organizations.
The PCN Team consists of ACPs, Nurses, Frailty Care Coordinators, Social Prescribers, Pharmacy Technicians, and Long-term Conditions Coordinators. Senior clinical advice will be provided by the Lead ACP, and all three practices have access to senior clinical support.
Main Duties of the Job
The post holder will provide an innovative, integrated service to patients living within the Carlisle PCN boundary who require further support. The post holder will work closely with the PCN Frailty Team and GP Practices to provide patient-centered care, primarily providing a Home Visiting Service. The role involves triaging and assessing patients with undifferentiated and undiagnosed problems, initiating treatment (including prescribing) and investigations, providing advice and management, and liaising with other health and care professionals as necessary.
The post holder will provide appropriate patient care within their scope of practice and in accordance with practice protocols and policies. The post holder will actively engage patients and carers in the management of their health, promoting health and wellbeing.
The post holder will be responsible for recognizing, negotiating, and implementing change processes to enhance the delivery of care to the Carlisle PCN population. The post holder will contribute to the development and delivery of a high-quality, innovative service to patients within the community through the implementation of evidence-based clinical practice, supported by continual professional development, as part of the wider multi-disciplinary team.

Job Responsibilities
Professional Role
- Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
- Maintain professional registration and keep up-to-date with mandatory training.
- Record accurate consultation data in patient records in accordance with the latest regulatory guidance and other pertinent standards.
- Proactively promote the role of the ACP, including externally to key stakeholders and agencies.
Clinical Role
- Undertake visits to patients as requested by the practices.
- Assess patients within their role and make professionally autonomous decisions.
- Provide a first point of contact alongside the Frailty team for Nursing and Residential Care Homes.
- Provide regular ward rounds and telephone calls/visits to Care Homes and anticipatory care visits to Housebound as per the post-holder's caseload.
- Instigate necessary diagnostic tests or investigations where appropriate and interpret findings/reports and act upon them appropriately. Review patients where needed and make long-term care plans.
- As an independent prescriber, prescribe safe, effective, and appropriate medication as defined by current legislative framework.
- Work with the medicines management team to provide and/or ensure polypharmacy medication reviews and de-prescribe where appropriate.
- Provide safe, evidence-based, cost-effective, individualized patient care.
- Provide advanced care and respect planning and End of life care where appropriate.
- Promote health and wellbeing through the use of disease prevention and screening, health promotion, and health education as part of an MDT.
- Refer patients directly to other services/agencies as appropriate.
- Work with the MDT to ensure practice and PCN contractual requirements are delivered and met for patients within caseload and in support of our wider population.
- Work with the MDT to ensure our care is compliant, safe, of high quality, and follows regulatory guidelines at practice and PCN level.
- Participate in the identification of community health needs around our population health data and the identification of health inequalities and develop patient, family, and/or community-centered strategies to address them as part of our MDT.
- Help develop and set up new patient services and participate in initiatives to improve existing patient services at PCN and locality level.
Training & Educational
- Currently enhancing professional development and maintain a personal development plan.
- Continually keep up-to-date with current evidence-based practice.
- Involved in lifelong learning.
- Participate in in-house training provided and completion of all mandatory training at clinical and practice levels.
- Keep up-to-date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.
- Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the PCN. It is the individual's responsibility to remain up-to-date with recent developments.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring PREP requirements are met.
- If necessary to expand the role to include additional responsibilities, full training will be given.
Individual Responsibilities
The post holder is expected to:
Adhere to Practice policies and procedures and relevant legislation, including the requirements of any professional bodies.
Attend mandatory training as identified by the PCN.
Confidentiality
The post holder must maintain the confidentiality of information about patients, staff, and Practice business in accordance with the General Data Protection Regulation, Caldicott principles, and Practice Information Governance policies and procedures.

Risk Management
Ensure that you implement systems and procedures at local level to fulfill the requirements of the Practice risk management strategy, including local management and resolution of compliance and concerns, management of untoward incidents/near misses, and compliance with the risk profiling programme.

Qualifications
Essential
- Current professional registration.
- Independent Non-Medical Prescriber V300.
- Evidence of recent Continual Professional Development.

- Education or courses relating to older people, frailty, and chronic disease management.
