Nurse in Community Care

As a Community Nurse with Northamptonshire Healthcare NHS Foundation Trust, you will make a significant difference in the lives of patients by providing high-quality nursing care in their own homes. We are seeking an experienced Registered Nurse who is passionate about delivering excellent care and has a strong background in acute medicine, Emergency Care, or community nursing.
This role offers a unique opportunity to work as part of a multi-disciplinary team, providing acute nursing assessments and interventions in patients' homes. You will be responsible for ensuring patients receive the necessary care and support to remain in their home environment, minimizing hospital admissions and re-admissions.

- Be professionally accountable for the delivery of care
- Utilize technology to update records as needed

Person Specification:
- NMC registered
- Experience in community nursing, venepuncture, palliative care, or UK car driving
