Job Summary:
We are seeking a highly skilled and compassionate Lung Oncology Support Worker to join our team at Colchester Hospital University NHS Foundation Trust. As a key member of our Lung Cancer Support Team, you will play a vital role in providing exceptional patient care and support to individuals diagnosed with lung cancer.
Key Responsibilities:
- Work closely with Clinical Nurse Specialists (CNS) to support patients and ensure personalized care.
- Provide support to patients through telephone calls, assisting with queries and signposting to appropriate healthcare professionals.
- Carry out holistic needs assessments and care planning, seeing patients in clinic and the chemotherapy day unit.
- Ensure patients' needs are met and they are aware of wellbeing services and support available.
- Refer patients for pre-habilitation and rehabilitation services as appropriate.
- Provide administrative support to the Lung Oncology CNS team, including data collection and analysis.
- Work independently, with close support from other teams involved in patient care.
Requirements:
- Ability to work as an integral part of the Lung Oncology team.
- Responsibility for the lung team telephone system, responding to messages and escalating to the CNS as necessary.
- Ability to accept and resolve telephone enquiries from patients regarding their care and treatment or generic enquiries.
- Triaging calls for the Clinical Nurse Specialist team and forwarding clinical queries to the appropriate Specialist Nurse or clinician.
- Providing administrative support to the Lung Oncology CNS team, wider MDT, and cancer network.
- Acting as a communication link between hospital, patients, and GPs.
- Being a key contact for all patients diagnosed with lung cancer and on the cancer pathway.
