Job Overview

Leicestershire Partnership NHS Trust is seeking a highly motivated and experienced Occupational Therapist to join our Community Health team. As a Band 6 Occupational Therapist, you will play a key role in delivering high-quality occupational therapy services to patients in their own homes.

The successful candidate will have a strong background in occupational therapy and excellent communication and interpersonal skills. You will be responsible for assessing patients' needs, developing and implementing individualized care plans, and working collaboratively with other healthcare professionals to achieve optimal outcomes.

Main Responsibilities
  • To provide high-quality occupational therapy services to patients in their own homes, including assessments, goal setting, and interventions.
  • To work collaboratively with other healthcare professionals, including physiotherapists and technical instructors, to achieve optimal patient outcomes.
  • To develop and implement individualized care plans that meet the unique needs of each patient.
  • To maintain accurate and up-to-date records of patient assessments, interventions, and outcomes.
  • To participate in audit and research projects to further improve clinical practice and patient outcomes.
Requirements
  • Registration with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
  • Postgraduate qualification in Occupational Therapy or a related field.
  • Experience of working in a community health setting, preferably with older adults or individuals with complex needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality patient care.
