We are seeking an experienced Person-Centered Care Worker to join our team, who shares our commitment to delivering high-quality, person-centered support services that cater to the unique needs and aspirations of our service users.

In this challenging and rewarding role, you will have the opportunity to work closely with service users, developing personalized care plans that promote independence, self-confidence, and positive relationships.

Your responsibilities will include:

  • Providing person-centered support to service users, tailoring your approach to meet their individual needs and preferences;
  • Liaising with other professionals, including healthcare teams, to ensure comprehensive support for our service users;
  • Developing and implementing care plans that promote independence, self-confidence, and positive relationships;
  • Fostering a supportive and inclusive environment, where service users feel valued and empowered to reach their full potential;

We offer a competitive salary range of £29,000 - £34,000 per annum, plus benefits package, which includes:

  • 31 days' paid holiday per year, increasing with each year of service up to 33 days;
  • The option to buy additional holidays and spread the cost;
  • A comprehensive benefits package, including employee discounts, pension scheme, and access to training and development opportunities.
