About the Role

We are seeking a highly skilled and dedicated Advanced Practitioner to join our North Locality Team in Adult Social Care. As a key member of our team, you will play a vital role in supporting adults with complex needs and promoting their independence.

Key Responsibilities
  • Carry out comprehensive needs assessments to identify individualized care plans
  • Develop and implement person-centered care plans in collaboration with service users, families, and other professionals
  • Provide expert guidance and support to colleagues and partners to ensure high-quality care delivery
  • Contribute to the development and implementation of policies and procedures to improve adult social care services
  • Participate in multidisciplinary teams to coordinate care and support services
Requirements
  • Postgraduate qualification in social work or a related field
  • Significant experience in adult social care, preferably in a leadership or advanced practice role
