The successful candidate will utilize their clinical expertise, leadership, and education skills to ensure a seamless service for patients, carers, and staff. They will provide advice, support, and information, working closely with the multidisciplinary team to deliver a coordinated quality service.

Main Responsibilities

To facilitate a safe and timely discharge for frail patients within ED, utilizing comprehensive geriatric assessment in collaboration with other team members.

Person Specification
  • Sound theoretical and practical knowledge relating to frailty and elderly care.
Requirements
  • Experience of implementing a change in practice
  • Experience in project management
Skills and Abilities
  • Effective verbal, written, and oral communication skills
  • Ability to design and deliver teaching effectively to small and large groups

Salary: £42,939 to £50,697 per annum inclusive
