Older Adults Liaison Practitioner

We are seeking a dedicated Adult Health Practitioner to join our Older Adults Liaison Psychiatry Team at Nottingham University Hospitals. This role offers a unique opportunity for professionals to develop advanced skills tailored to the needs of older adults.
In this position, you will be responsible for delivering comprehensive mental health assessments, interventions, and care planning for older adults presenting with complex psychiatric and physical health needs.
You will also collaborate with acute hospital colleagues, providing expert advice and consultation to ensure seamless care for patients with mental health conditions.
The successful candidate will have a professional mental health nursing qualification and experience working within multidisciplinary teams. They will also have recent experience with older adult mental health and community mental health experience working with older people with mental health problems.
