About the Role

Sonderwell is seeking a highly skilled and experienced Clinical Lead Nurse to join our team in the Hereford and Worcester region. As a Clinical Lead Nurse, you will be responsible for leading, supporting, and facilitating the nursing and medical needs of service users with their commissioned care.

Key Responsibilities
  • Client-Centred Care: Deliver personalized care outcomes, conduct care assessments, and develop care plans that meet the unique needs of each service user.
  • Leadership and Support: Collaborate with the Senior Nurse Manager and Registered Manager to provide leadership and support to care and support staff, promoting a culture of excellence and quality care.
  • Professional Development: Maintain your own professional skills and knowledge, undertaking required training and development to ensure you remain up-to-date with best practices and industry standards.
  • Compliance and Quality: Ensure that all care delivery meets national standards, legislative requirements, and organizational policies, promoting a culture of quality and compliance.
What You'll Do
  • Develop Individualized Care Plans: Work with service users, families, and healthcare professionals to develop care plans that are patient-focused, realistic, and achievable.
  • Monitor Client Outcomes: Proactively monitor and evaluate client outcomes, using your expertise to guide and implement changes that improve care delivery.
  • Initial Assessments: Conduct initial holistic nursing assessments in partnership with service users, families, and carers, involving other statutory, voluntary, and third-party sector providers as necessary.
  • Day-to-Day Oversight: Provide day-to-day oversight and management of the regional care delivery team, working with committed carer teams to maintain all required standards.
  • Recording and Documentation: Ensure that all recording systems and organizational documentation are of a high standard and up-to-date.
  • Team Management: Motivate and manage your nursing and care team, embedding the company's values and behaviors and promoting a culture of pride in their work.
  • Reporting and Improvement: Report against targets and standards, putting in place action plans for service improvements that have a positive impact on clients, service quality, and business development.
Working with Others
  • Collaboration and Support: Work in collaboration with Central Support, including HR, to motivate carers and nurses through informal check-ins, formal supervision, and appraisals.
  • Rota Development: Oversee the development and implementation of rotas, ensuring that field staff are appropriately trained, assessed, and signed off as competent prior to lone working.
  • Service User Welfare: Be responsible for promoting and protecting the welfare of our service users.
  • Interdisciplinary Collaboration: Work in cooperation with members of the multidisciplinary teams to maximize opportunities for our service users, identifying information, concerns, and ideas for improvement and sharing them with the senior management team of Sonderwell.
