Job Summary:
We are seeking an experienced Clinical Lead Nurse to join our team at Sonderwell. As a Clinical Lead Nurse, you will be responsible for leading, supporting, and facilitating the nursing and medical needs of service users with their commissioned care. You will work closely with a team of experienced carers and regional nurses to deliver outstanding care in the community.
Key Responsibilities:
Delivering personalised client-centred service outcomes, and carrying out care assessments and writing care plans and managing competencies.
Positively supporting the Senior Nurse Manager and the Registered Manager in providing leadership to the care and support staff.
Maintaining your own professional skills to your current level and undertaking required training and development for personal professional development.
Performing in accordance with national standards, following legislative requirements, relevant regulations, and accepted best practice.
What You'll Do:
Develop individualised care plans in partnership with the service user, family, and other healthcare professionals as appropriate, ensuring goals are patient-focused, realistic, and achievable.
Monitor client and family-centred outcomes, proactively using your expertise to guide and implement changes.
Undertaking initial holistic nursing assessments in partnership with the Service User, family, and carer and involving other statutory/voluntary/third 'party' sector providers where necessary.
Day-to-day oversight and management of the regional care delivery team, working with committed carer teams to maintain all required standards.
Ensuring all recording systems and organisational documentation are of a high standard and up to date.
Motivating and managing your own nursing/care team, embedding the company's values and behaviours and a culture of 'pride' in their work.
Managing and supporting field-based care support workers and nurses through regular supervisions, competency assessments, and appraisals.
Reporting against targets and standards; putting in place action plans for service improvements that have a positive impact on clients, service quality, and business development.
Having a strong commitment to quality standards to ensure compliance to statutory, mandatory, and contractual requirements.
Working with Others:
Working in collaboration with Central Support, including HR, to motivate carers and nurses through informal check-ins, formal supervision, and appraisals.
Overseeing the development and implementation of the rota's by ensuring that the field staff are appropriately trained, assessed, and signed off as competent prior to lone working.
Being responsible for promoting and protecting the welfare of our service users.
Working in cooperation with members of the multidisciplinary teams to maximise opportunities for our service users. Identify information, concerns, and ideas for improvement and share them with the senior management team of Sonderwell.
About You:
The ideal candidate will have experience of working in the community, tracheostomy, peg, and catheter care, and passing on their knowledge to others in the package. You will be happy working with both adults and paediatric care, and we will provide training where required. We are unable to provide sponsorship for this role, so you must have the full right to work in the UK at this time.
