Community Health Nurse

We are seeking a skilled and dedicated Community Nurse to join our Felixstowe Community Healthcare Team. As a Community Nurse, you will play a vital role in providing high-quality nursing care to housebound residents in Felixstowe and surrounding areas.
Main Responsibilities- Triage, assess, plan, implement, and evaluate all aspects of nursing patient care, developing care plans that meet individual needs.
- Ensure informed consent is obtained and documented prior to initiating interventions.
- Develop and implement individualized, evidence-based care plans for patients, modifying them as required.
- Assess patients' holistic needs, communicating complex and sensitive information to patients and carers.
- Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care coordinated.
- Maintain skills with regards to latest technology and participate in multi-disciplinary/multi-agency meetings as appropriate.
- Ensure own workload is planned and prioritized according to patient needs.
- Ensure patient-held records are completed for each visit, and SystmOne inputting is completed daily.

Requirements- Current NMC registration
- RN Qualification (Part 1 of register) or equivalent
- Car Driver and have access to a car for work which is community based
- Leg Ulcer Management
- IV Administration / cannulation
- ENB 998/mentorship preparation or equivalent
- Mentorship qualification is essential for the post, but not essential at interview
- Previous experience working in the community
