Key Responsibilities:

* Provide support and guidance to young people in their homes
* Help young people develop independence skills and achieve their goals
* Work closely with social workers and external agencies to ensure the best possible outcomes for young people
* Maintain accurate records and reports
* Participate in training and development opportunities to enhance skills and knowledge

Requirements:

* NVQ (QCF) Level 3 (or above) in Health & Social Care
* Minimum of 12 months experience working with vulnerable young people
* Excellent written and spoken English
* Current DBS check
* Full clean, UK driving licence
* IT literacy - MS Office
* Excellent communication, organisation and report writing skills
* Flexible, empathic, supportive attitude
* Ability to work autonomously
* Training in safe-guarding, child protection, risk assessments beneficial
