Care Coordinator

We are seeking a highly skilled and dedicated Registered Mental Health Nurse (RMN), Registered Learning Disability Nurse (RNLD) or Registered General Nurse (RGN) to join our expert multidisciplinary team in a Specialist Care Home located in Cardiff that supports adults with complex needs.
The successful candidate will be responsible for providing holistic resident assessments, developing tailored care plans, and working collaboratively with the multi-disciplinary team to ensure effective care strategies. This role offers flexible working options, including part-time and full-time, as well as specialist skills training, apprenticeships, and nationally recognised qualifications.
The salary for this position is £35,100 - £40,950 per annum based on 37.5 hours per week, approximately £18 - £21 per hour. In addition to a competitive salary, we offer a range of benefits, including discounts on supermarkets, high street brands, and gym memberships, as well as mileage reimbursement at 45p per mile for work travel.
