About the Role

We are seeking an experienced Care and Support Professional to join our team at Elysium Healthcare. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with Learning Disabilities and Autism.

Your primary responsibility will be to develop and implement personalized care plans that meet the unique needs of each individual. You will work closely with our team to ensure seamless delivery of care services, maintaining accurate records and reports as required.

Key Responsibilities:

  • Develop and implement personalized care plans in collaboration with the multidisciplinary team
  • Provide high-quality care and support to individuals with Learning Disabilities and Autism
  • Work closely with colleagues to ensure seamless delivery of care services
  • Maintain accurate records and reports as required
