Job Title: Community Support Worker - Older Adult Mental Health

We are seeking a highly motivated and compassionate Community Support Worker to join our Memory Team in Scarborough. As a key member of our team, you will play a vital role in providing high-quality care and support to older adults with concerns about their memory.

The successful candidate will have a Diploma in Health and Social Care Level 2 or equivalent, a Care Certificate, and Key skills in Literacy and Numeracy level 2. They will also have previous experience as a Band 2/3 HCA or equivalent in a care environment, as well as experience of working with people with mental/social health needs or learning disabilities.

Main Responsibilities:
  • Provide a range of services including keeping in touch calls with service users and families prior to first contact with the team
  • Support with physical health checks, both prior to and after prescribing where appropriate
  • Practical and emotional support to individual service users, their carers and families as indicated by the individual care plans co-created by the person and their Link Worker
Requirements:
  • Ability to travel independently in accordance with Trust policies and service need
  • Effective verbal and written communication skills
  • Time management and ability to prioritise effectively
  • Competency in record keeping (within agreed timescales)
  • Competency in ECG and venepuncture
Person Specification:
  • Diploma in Health and Social Care level 2 or equivalent
  • Care Certificate
  • Key skills in Literacy and Numeracy level 2
  • Successful completion of Trust's full HCA Framework within 6-month probationary period
