Job Title: Care and Community Services Officer

We are seeking a highly motivated and compassionate Care and Community Services Officer to join our team at Tees Esk and Wear Valleys NHS Foundation Trust. As a key member of our Memory Team, you will play a vital role in providing high-quality care and support to older adults with concerns about their memory.

Main Responsibilities:
  • Provide a range of services including keeping in touch calls with service users and families, support with physical health checks, and practical and emotional support to individual service users, their carers and families.
  • Work collaboratively with a multi-disciplinary team of practitioners, including Nurses, Psychiatrists, and Psychologists, to deliver a high standard of care and support.
  • Support professionally qualified staff to implement, monitor and evaluate person-centred, co-created plans of care.
Requirements:
  • Diploma in Health and Social Care level 2 or equivalent.
  • Care Certificate.
  • Key skills in Literacy and Numeracy level 2.
  • Successful completion of Trust's full HCA Framework within 6-month probationary period.
Desirable Qualities:
  • Previous experience of working with older adults in a health/social care setting.
  • Experience of supporting older adults with physical and/or mental health problems in the community.
Working Arrangements:

This post is subject to a satisfactory Enhanced Disclosure and Barring Service check. The successful candidate will be required to travel independently across the locality and have access to a suitable vehicle for business purposes.
