About the Role

We are seeking a highly experienced and skilled leader to join our team as the Head of Community Services. This is a key leadership role, responsible for overseeing and developing our community-based services in line with the hospice's strategic objectives and values.

Key Responsibilities
  • Provide leadership and management across a range of hospice community services, including the Clinical Nurse Specialist team, domiciliary care services, wellbeing initiatives, and future outreach programs.
  • Work closely with the Director of Clinical Services to ensure that the hospice meets the needs of patients and their families, particularly in community settings.
  • Lead and manage multidisciplinary teams to deliver high-quality, evidence-based care.
  • Ensure clinical governance, audit processes, and data collection meet statutory and organisational standards.
  • Champion excellence in palliative care, ensuring that services meet the needs of the local community.
  • Actively contribute to the development and implementation of the hospice's overarching strategy.
