Job Title: Band 6 Community Psychiatric Nurse/ Registered Mental Health Nurse

Central and North West London NHS Foundation Trust is seeking a skilled and compassionate Band 6 Community Psychiatric Nurse/ Registered Mental Health Nurse to join our Community Mental Health Team. As a key member of our team, you will play a vital role in providing high-quality care to individuals experiencing mental health issues, particularly those who are homeless or rough sleeping.

Job Summary:

We are looking for a dedicated and experienced mental health professional to work with our team to deliver an assertive and responsive service to rough sleepers and homeless individuals. The successful candidate will have a strong understanding of mental health nursing principles and practices, as well as experience working with vulnerable populations.

Key Responsibilities:
  • Conduct comprehensive assessments of mental health, housing, and social care needs for clients
  • Develop and implement individualized care plans to address client needs
  • Provide short-term interventions to link clients into mainstream services
  • Collaborate with other statutory services, housing providers, and social care teams to deliver a comprehensive service
  • Participate in regular reviews of team performance and goal setting
Requirements:
  • RMN with current NMC registration
  • Experience working in a setting that provided care for people who were acutely and chronically ill
  • Understanding of current government policy in relation to mental health, homelessness, and rough sleeping
  • Ability to work in a fast-paced environment and prioritize tasks effectively
