We are Unity Healthcare Recruitment Ltd, a leading provider of healthcare services, and we are seeking a skilled Clinical Nurse Specialist to join our team in Sussex.

The ideal candidate will have experience in delivering high-quality nursing care to patients in community settings, with a strong focus on patient-centred care and promoting independence.

This role involves working autonomously, managing a daily caseload, and prioritizing tasks to ensure safe and effective care delivery. The successful candidate will be required to work collaboratively with other healthcare professionals, including GPs, therapists, and social workers, to ensure coordinated care.

We offer a competitive salary of £35,000 - £45,000 per annum, depending on experience, and a range of benefits, including flexible working hours, access to continuing professional development (CPD) pathways, and a supportive and inclusive working environment.

Key Responsibilities:

  • Clinical Assessment and Care Planning:
    • Conduct holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
    • Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
  • Community-Based Care:
    • Work autonomously in patients homes and community settings, ensuring safe and effective care delivery.
    • Manage a daily caseload, prioritising tasks and responding to changes in patients conditions.
  • Team Collaboration:
    • Work closely with other healthcare professionals, such as GPs, therapists, and social workers, to ensure coordinated care.
    • Participate in multidisciplinary team meetings and case conferences to support care planning and review.
  • Professional Development:
    • Commit to continuing professional development (CPD) to maintain and enhance clinical skills and knowledge.
    • Participate in clinical supervision, reflective practice, and training sessions provided by the organisation.
  • Compliance and Governance:
    • Maintain accurate and timely clinical records in accordance with professional and organisational standards.
    • Adhere to local and national policies, guidelines, and regulations, including infection control and safeguarding protocols.

Person Specification:

Essential Criteria:

  • Registered Nurse (Adult) with current NMC registration.
  • Clinical experience in a relevant setting, such as community, primary care, or acute services.
  • Ability to work independently and manage a caseload in a community environment.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication, interpersonal, and organisational skills.
  • A full UK driving licence and access to a vehicle for work purposes (if applicable).
  • A commitment to patient-centred care and promoting independence.

Desirable Criteria:

  • Previous experience in community nursing.
  • Knowledge of long-term condition management, palliative care, or wound care.
  • Completion of relevant post-registration training (e.g., venepuncture, IV therapy, or non-medical prescribing).
