Reablement Support Specialist

About the Role
Cheshire East Council is seeking a skilled Reablement Worker to join their team. The successful candidate will be responsible for providing short-term support to individuals to help them achieve maximum independence with personal care and daily living skills.
The role involves working in a person-centred manner to support discharges from hospitals, complete assessments, and reable individuals back to independence or for ongoing care needs and rehabilitation of therapy exercises.
The team also supports hospital avoidance by working alongside the Urgent Community Response/Virtual Ward teams to provide a same-day response over a 72-hour period when someone has a health crisis and is supported at home rather than being admitted to hospital when safe to do so.
About You
To be successful in this role, you will need to have effective teamwork and excellent communication skills, as well as an enthusiastic, flexible, and reliable approach to work.
You will also need to be empathetic and compassionate, able to work on your own initiative and as part of a team, and be person-centred in your approach to achieving the best outcomes for people.
