Are you a motivated and caring Social Worker looking for a new challenge? We have an exciting opportunity available for a Community Care Specialist to join our team at iPeople Solutions in Exeter, Devon.
As a key member of our Adult Social Services team, you will be responsible for providing expert support to vulnerable adults aged 18 and above, focusing on Older People and Learning Disability needs.
Your responsibilities will include:
- Developing and implementing comprehensive care plans that meet individual needs
- Working collaboratively with healthcare professionals and other stakeholders to ensure seamless care delivery
- Maintaining accurate records and reports, adhering to regulatory standards
- Providing ongoing support and guidance to service users, empowering them to make informed decisions about their care
