Senior Healthcare Practitioner

We are seeking a Senior Healthcare Practitioner to join our team at Midlands Partnership NHS Foundation Trust.
This is an exciting opportunity for a skilled professional to work in unplanned care settings, providing high-quality support to adults with complex needs associated with older age, physical health or mental health needs and their carers.
The successful candidate will have experience of working as part of a multi-disciplinary team and be able to develop and implement effective care plans. They will also have excellent communication and interpersonal skills, enabling them to build strong relationships with colleagues, service users, and their families.
Main Responsibilities- To undertake statutory assessments in complex situations, using knowledge of Mental Capacity and Continuing Health Care legislation.
- To work collaboratively with other healthcare professionals to provide comprehensive care plans that meet the needs of service users.
- To communicate effectively with service users, their families, and other stakeholders to ensure that their needs are met and their voices are heard.
- A Professional Social Work qualification (or equivalent).
- Registration with the Professional Social Work regulatory body.
- Approved Mental Health Professional and/or Best Interest Assessor status.
- Good keyboard/IT skills.
- Significant post-qualification experience of working with adults with complex needs.
The successful candidate will be required to work within a fast-paced environment, prioritizing tasks and managing multiple responsibilities. They will also be expected to maintain accurate records and contribute to the development of policies and procedures.
