Job Title: Young Person's Resilience Worker - Supported Accommodation Lead

We are seeking a highly skilled and compassionate individual to join our team as a Young Person's Resilience Worker - Supported Accommodation Lead. In this role, you will be responsible for delivering high-quality provision to young people in supported accommodation, utilizing a young person-centered approach to empower them in reducing risk, enhancing resilience, and leading safe, happy, and healthy lives.

About the Role:

This is an exciting opportunity to make a significant impact in the lives of young people. As a Young Person's Resilience Worker - Supported Accommodation Lead, you will be an integral part of our multi-disciplinary team at Positive Futures Young Person service, working closely with service users, colleagues, and external partners to provide comprehensive care and support.

Key Responsibilities:
  • Assist service users from the moment they enter the service through their treatment and recovery journey.
  • Conduct screening, assessments, and psychosocial interventions.
  • Develop integrated care plans and offer post-treatment support, including transfer of care.
  • Identify and respond effectively to potential safeguarding issues.
  • Focus on reducing drug and alcohol-related harm, especially among young people in supported accommodation.
  • Develop therapeutic relationships and deliver a personalized package of care, including evidence-based psycho-social interventions.
  • Engage in 1-to-1 key working and group work across supported accommodation sites in Gateshead.
  • Implement harm minimization and brief interventions.
  • Contribute to whole-family interventions.
  • Work closely with your service's Safeguarding Lead to ensure swift and effective resolution of issues and concerns.
  • Establish relationships with emotional well-being and mental health providers to enhance pathways in and out of service.
  • Provide advocacy for young people by actively participating in relevant multiagency meetings, including Child Protection meetings, Complex Case Review panels, and Team Around the Child meetings.
About You:
  • Experience engaging with young people individually and in groups.
  • Understanding of modern youth issues and safeguarding concerns.
  • Knowledge of drug/alcohol issues and strong partner relationship skills.
  • Excellent time management, initiative, and collaboration abilities.
