This is an exciting opportunity to work as a Clinical Care Specialist with Central London Community Healthcare NHS Trust. As a member of our team, you will provide high-quality care to patients in their own homes, focusing on preventing hospital admissions and facilitating early discharge.

Your key responsibilities will include working autonomously and collaboratively with other healthcare professionals to deliver excellent standards of patient care. You will be required to prioritise and manage your workload with minimal supervision, ensuring that you are always available to provide direct advanced clinical care to patients presenting with undifferentiated and complex health needs.

Requirements:
