Job Summary

We are seeking a skilled and dedicated Practice Nurse to join our team at Nantwich Health Centre. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our practice.

Main Responsibilities

The successful candidate will be responsible for:

  • Providing clinical care to patients, including chronic disease management, cervical screening, and immunisations.
  • Working collaboratively with the general practice team to meet the needs of patients.
  • Developing and implementing care plans to improve patient outcomes.
  • Mentoring and supporting junior staff members.
  • Participating in quality improvement initiatives and audits.
