Job Title: Community Healthcare Assistant

Hounslow and Richmond Community Healthcare NHS Trust is seeking a skilled Community Healthcare Assistant to join their team on a temporary basis. The successful candidate will work closely with the District Nursing Team to provide high-quality care and support to patients in the community.

Key Responsibilities:
  • Carry out clinical duties and undertake clinical skills as per the needs of the service and specific patient after training and competence assessment with support from an identified mentor.
  • Liaise with other healthcare and multi-agency workers to provide a multi-disciplinary approach to care.
  • Meet at agreed times to discuss patient care and collect allocated worklist from District Nurse Team Leader or Community Staff Nurse, and take referrals from other professions as agreed, reporting back to caseload holder.
  • Demonstrate, teach and supervise simple procedures that can be safely delegated to patient/carer with support and approval from the District Nurse Team Leader or Community Staff Nurse, as necessary.
Requirements:
  • GCSE grade A – C in English or Maths.
  • Commitment to providing a high-quality, professional, caring service to all patients.
