We are seeking a skilled and compassionate Rehabilitation Support Worker to join our Early Supported Discharge team. As a key member of our multidisciplinary team, you will work closely with patients who have recently been discharged from hospital, providing high-quality rehabilitation support and promoting independence.
Main Duties
- Support qualified therapy staff in implementing and undertaking designated patient assessment and treatment programmes.
- Document, evaluate, and feedback to qualified therapists following all interventions.
- Assist the ESD team with non-clinical tasks.

Person Specification
Essential Criteria
- Level 3 GCSE or equivalent in English.
- NVQ level III in Health Care Manual Handling.
Desirable Criteria
- NVQ level III in Health Care Manual Handling.
- Life Saving or First Aid qualification.

- Clear, concise verbal and written communication to ensure liaison with multi-disciplinary teams, patients, and carers.

- Physical ability and endurance to work in the clinical area required by this post.
Special Circumstances

- Driving licence and ability to drive/travel on occasions to other locations in Bucks and other FNP sites.
