Community Nursing Specialist

This is an exciting time for Calderdale and Huddersfield NHS Foundation Trust's community nursing teams. Our district nursing service provides high-quality, evidence-based care to patients in their own homes. As a skilled nurse practitioner, you will be responsible for planning, delivering, and evaluating patient care within the trust's specialist teams.
The ideal candidate will hold a full driving license with access to a suitable vehicle for business use. We have fixed-term vacancies in Central Halifax to cover maternity leave for 12 months. The successful applicant will work closely with our expert teams to develop and implement programmes of care that meet the needs of our patients.
We are seeking a qualified community staff nurse who is passionate about providing the best possible care and empowering patients to take control of their health. You will gain experience in a wide range of clinical skills and participate in ongoing training and development opportunities.
Main Responsibilities- Develop and deliver high-quality patient care plans.
- Participate in the assessment, development, implementation, and evaluation of programmes of care.
- Work collaboratively with multidisciplinary teams to achieve excellent patient outcomes.
- NMC Registered Nurse in relevant field of practice.
- Additional post-registration qualification or evidence of continuing professional development.
- Evidence of introducing change and innovation into nursing practice.
