Job Summary

Newton Place Surgery is seeking a highly skilled Diabetes Nurse Specialist to join our dynamic team. As a key member of our clinical team, you will be responsible for providing high-quality, evidence-based diabetes care to patients with complex needs.

Main Responsibilities
  • Work autonomously to provide specialist diabetes care to patients from diagnosis to long-term management.
  • Initiate and support insulin management and/or other injectables for complex type 1 and 2 diabetic patients.
  • Promote self-care management to patients to prevent worsening of their condition and improve health outcomes.
  • Liaise with administration staff to ensure smooth running of clinics and accurate record keeping.
  • Maintain accurate data and records using EMIS and other software as applicable.
  • Ensure full understanding of QOF for diabetes chronic disease management and liaise with the team to achieve targets.
  • Act as a specialist resource and provide advice to patients, family/carers, and health professionals.
About Us

Newton Place Surgery is a purpose-built surgery with a great family feel, located in the beautiful market village of Faversham in Kent. We have good transportation links and are part of the Mid Kent PCN, one of the largest PCNs in East Kent.

We offer a variety of additional on-site services, including ultrasound, audiology, physiotherapy, and minor surgery. Our practice is actively involved in research and employs a small team to support this. We are also a training practice that hosts ST2, ST3, medical students, PAs, paramedics, and student nurses.

Person Specification
  • Post-registration experience as a qualified nurse.
  • Extensive knowledge of diabetes, preferably in a Diabetes Specialist Nurse or practice nurse role with experience running Diabetes clinics independently.
  • Level 1 NMC Adult Registered Nurse qualification.
  • Relevant post-registration qualification and Diabetes Diploma/Certificate.
  • Desirable: Non-Medical Independent Prescriber or willingness to undertake.
