As a key member of our team, you will play a vital role in providing high-quality rehabilitation care to patients with specialist needs.

Main Responsibilities
  • Assess, plan, implement, and evaluate care for assigned patients, ensuring a high standard of rehabilitation care.
  • Deliver evidence-based care, utilizing excellent communication skills and a supportive approach to team working within a rehabilitation setting.
  • Structure your own learning, supported by the team, to gain new knowledge and skills that benefit yourself and the service.
  • Work collaboratively with families and colleagues in complex discharge planning and best interest discussions.
  • Contribute actively to the rehabilitation process, working alongside therapy colleagues to set joint goals with patients and ensure the whole team is aware of the goals to focus on and achieve.
