Job Description: Care and Support Specialist
Lifeways is seeking a dedicated Care and Support Specialist to join our team. The successful candidate will provide person-centred care and support to individuals with complex needs, promoting independence and well-being.
Responsibilities:
- Develop and implement individualised support plans
- Provide emotional and practical support to service users and their families
- Collaborate with healthcare professionals to ensure seamless care transitions
- Manage and maintain accurate records of service user information
- Participate in staff meetings and contribute to team decision-making
Requirements:
- Minimum 6 months' experience working in a care or support setting
- Strong communication and interpersonal skills
- Ability to build positive relationships with service users, families, and colleagues
- Good observational skills and ability to accurately record incidents
Estimated Salary: £14.75 per hour, based on location and experience
