About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team at Cheshire and Wirral Partnership NHS Foundation Trust. As a key member of our Intensive Support Service, you will work closely with patients with complex mental health needs, providing high-quality care and support to help them achieve their recovery goals.

Key Responsibilities
  • Deliver individualized care plans, working closely with patients, families, and multidisciplinary teams to ensure effective and adaptive patient care.
  • Provide high-quality assessments, interventions, and support to patients with complex mental health needs, promoting independence and recovery.
  • Develop and implement specialist programmes of care, care packages, and provide specialized advice in relation to care.
  • Work closely with commissioners and care providers to ensure seamless care coordination and delivery.
