About the Role

In this role, you will be working closely with multidisciplinary teams to assess and develop personalized care plans for service users. You will also be responsible for managing caseloads, undertaking risk assessments, and ensuring that service users receive the necessary support and interventions to achieve their goals.

Key Responsibilities

  • Assessing and developing care plans in collaboration with service users, carers, and other professionals.
  • Managing caseloads and prioritizing tasks to ensure effective time management.
  • Undertaking regular risk assessments to identify potential risks and take appropriate action to mitigate them.
  • Providing information, advice, and support to service users, carers, and colleagues.
  • Collaborating with external agencies and organizations to ensure seamless care coordination.

Requirements

To be successful in this role, you will need to have:

  • A professional social work qualification (DipSW or equivalent).
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience of working in mental health services, preferably in a care coordination role.
  • Knowledge of legislation, theory, and policy related to mental health care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
