About the Role

We are seeking a highly skilled Community Healthcare Specialist to join our team at Hampshire and Isle of Wight Healthcare NHS Foundation Trust. As a key member of our Community Nursing Team, you will play a vital role in delivering high-quality care to patients across the Isle of Wight.

Key Responsibilities
  • Deliver individualized patient-centered care using a holistic approach.
  • Carry out delegated routine assessments and interventions spanning nursing, physiotherapy, and occupational therapy domains.
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
Requirements
  • Nursing Associate qualification registered with the NMC or equivalent.
  • Foundation Degree in Health & Social Care Practice or relevant health/social care degree.
  • Care certificate and/or relevant experience in a healthcare setting.
