Job Title: Therapy Services Coordinator

About the Role
We are seeking a highly organized and motivated individual to join our community team as a Therapy Services Coordinator. As a key member of our multidisciplinary team, you will work closely with consultants, specialist GPs, nurses, clinical pharmacists, physiotherapists, occupational therapists, therapy technicians, and HCAs to manage patients in the community for up to 6 weeks.

Key Responsibilities
• Coordinate therapy services across the community, ensuring seamless communication between teams.
• Manage patient data and maintain accurate records, ensuring compliance with National regulations for data protection and record keeping.
• Develop and implement strategies to improve therapy service delivery, promoting efficient use of resources and enhancing patient outcomes.

Requirements
• Excellent organizational and communication skills.
• Ability to interpret National regulations for data protection and record keeping and develop local systems to ensure these are maintained.
• Experience in coordinating healthcare services, preferably in a community setting.
