Mental Health Care Coordinator

We are seeking a highly skilled and dedicated Preceptorship Nurse to join our team at Jupiter Recruitment. As a key member of our mental health service, you will play a vital role in supporting patients in their recovery journey.
Key Responsibilities- Deliver High-Quality Patient Care: Provide prescribed care to a defined group of clients/patients within an agreed framework, taking into account patient choice and wishes.
- Develop and Implement Care Plans: Plan and implement a structured approach to nursing care, accurately observing and engaging with patients to assess their needs and evaluate progress.
- Collaborate with the Multi-Disciplinary Team: Contribute to a team approach to patient care, working closely with all members of the team to achieve the best possible outcomes.
- Monitor and Review Care Plans: Regularly review and update care plans to ensure they remain relevant and effective.
- Communicate Effectively: Develop and maintain effective relationships with patients, families, and carers, responding to suggestions and complaints in a professional and timely manner.
- Qualifications and Experience: Be qualified as an RMN or RNLD Nurse with a current active NMC Pin.
- Skills and Knowledge: Possess knowledge of NMC guidelines and professional practices, with the ability to develop and use flexible and innovative approaches to practice.
- Communication and Interpersonal Skills: Demonstrate excellent verbal and written communication skills, with the ability to work effectively in a team environment.
