About the Role
We are seeking a highly skilled and compassionate Care Coordinator to join our team at Rural Warwickshire North PCN. As a Care Coordinator, you will play a vital role in delivering high-quality, patient-centred care to our patients.
Key Responsibilities
- Work closely with our multidisciplinary team to identify patients' needs and develop personalised care plans.
- Provide coordination and navigation for patients and their carers across various healthcare services.
- Support local Care Homes to manage patient needs, including completing and reviewing Care Home Care Plans and Dementia reviews.
- Undertake assistance with clinician visits to patients in their own homes, where applicable.
- Make appropriate third-party referrals and complete accurate patient records.
Person Specification
- Ability to manage and prioritise a caseload.
- Excellent communication and interpersonal skills.
- Ability to build relationships with patients, their families, and carers.
- Ability to provide leadership and complete tasks in a timely manner.
- Full UK driving licence and access to a car.
