About the Role

Rural Warwickshire North Primary Care Network (PCN) is seeking a skilled Paramedic to join our team. As a Paramedic in our network, you will play a vital role in supporting the practices in expanding services available to our patients.

We offer a range of services, including Social Prescribing, Physiotherapists, Care Coordinators, and Clinical Pharmacists, to make healthcare more accessible to our patients. This is an exciting opportunity to make a real difference in primary care and support our patients.

The role will focus on supporting practices and patients in receiving the right care the first time around. You will work closely with GPs and other primary care professionals within the PCN to deliver a PCN model that reflects local priorities, health inequalities, and population health management.

Main Duties

The main duties of a PCN Paramedic include:

  • Working as part of a multi-disciplinary team (MDT) within RWNPCN
  • Advising patients on general healthcare and promoting self-management where appropriate
  • Performing specialist health checks and reviews within their scope of practice
  • Supporting the delivery of anticipatory care plans and leading certain services
  • Providing an alternative model to urgent and same-day GP home visits for the practice
  • Communicating proactively and effectively with all colleagues across the MDT
  • Maintaining accurate and contemporaneous health records

About Us

We are a Primary Care Network (PCN) of six practices working together with local providers to offer personalized, coordinated health and social care to our patients. Our PCN comprises 49,348 patients across the six practices. We aim to protect, improve, and extend local General Practice through greater organizational scale and innovation in service provision.
