Paediatrics Diabetes Care Manager

The successful candidate will be responsible for:
- Developing and implementing programmes of care for patients with specialist diabetes needs.
- Collaborating with community teams, including Respiratory, Liver, Cardiovascular, and Kidney Health Teams, to ensure seamless care delivery.
- Providing clinical expertise and professional guidance to multidisciplinary teams, patients, and carers within the project.
- Maintaining a defined caseload and setting standards of care through ongoing assessment and evaluation.
