An exciting opportunity has arisen for a skilled Registered Mental Health Nurse (RMN) to join Gray Healthcare Ltd's dynamic team. As a Community Nurse Advisor, you will play a pivotal role in providing high-quality care to individuals with complex needs and disabilities.

About the Role:
This position involves working closely with the multi-disciplinary team to assess, plan, and implement individualized support plans. You will be responsible for building relationships with service users, their families, and healthcare professionals to ensure effective care coordination and outstanding outcomes.

Key Responsibilities:
- Assess the needs of service users and develop tailored support plans
- Work collaboratively with the multi-disciplinary team to ensure seamless care coordination
- Build strong relationships with service users, their families, and healthcare professionals
- Ensure that all care plans are up-to-date and reflect changing needs and circumstances

Requirements:
- Current NMC registration as an RMN
- Minimum 2 years post-registration experience in the UK
- Experience of managing a caseload and developing therapeutic relationships
- Strong communication and interpersonal skills
