About the Role: This is an exciting opportunity to join our Health and Local Crisis Response service as a Service Support Worker. As a member of our team, you will play a vital role in supporting service users who may be struggling to do things for themselves due to age, illness, or vulnerability. Your key responsibilities will include providing reactive, focused, and flexible support to adults following a hospital stay, responding to referrals from Clinicians and Community Health Professionals, assessing service users' needs, and completing and following a support plan for the individual.

The Ideal Candidate: To be successful in this role, you will need to have a caring, patient, and helpful nature, as well as a good knowledge of services provided by the NHS and Social Care. You will also need to be able to make things great by improving service quality for the benefit of users, be professional, diplomatic, and confidential in your dealings with queries, and have a full UK manual driving licence. If you are passionate about making a difference in people's lives and have a desire to work in a fast-paced and dynamic environment, we would love to hear from you.
