Social Services Professional

We are seeking a skilled Adult Care Specialist to join our team in South Derbyshire. As a key member of our Adult Social Work Team, you will work closely with adults who have care and support needs, providing person-centred assessments and reviews.
Key responsibilities will include carrying out Care Act assessments and reviews, arranging services to meet eligible needs, and working in a person-centred and strengths-based way with clients and their families. You will also be responsible for undertaking safeguarding enquiries, capacity assessments, and working with other organisations and professionals.
As a Community Support Worker, you will provide a safe, accountable, and efficient service to our clients, helping them to achieve their outcomes. You will work closely with partner agencies, including GP surgeries, to promote effective multi-disciplinary practice.
